What is Health Passport?

Health Passport is a tool that contain essential demographic and medical information of an individual in a precise and clear format. It contains typical information that emergency healthcare providers could use to treat a patient.

Health URL is a short unique URL to access the medical information of an individual like pre-existing medical conditions, allergies,medications. This could help a health provider to access the patient’s health information in a secured way and render better care.

Creating your Health Passport

Anyone can create their health passport by following below easy steps

Steps to create health passport

1. Go to https://www.healthpassportonline.com/
2. Click the “SIGN UP” link
3. Enter the required details in the registration form and click create passport button
4. You have created your passport and you can start managing your health info

Managing your Health Passport

Once you have created you health passport account you can access your health information from anytime ,anywhere

Steps to access your health passport :

1. Go to https://www.healthpassportonline.com/
2. Click ‘Login’ button and enter your credentials
3. You will land in your dashboard. Start creating your health information and save the details using ‘Each section’s save button.
4. Once you fill all your health information and saved the changes you are ready to distribute your health info and do better care

Generating Health Passport URL

Health URL is unique URL that can be generated from your health dashboard ‘Generate URL’ menu available in the top menu

This is a unique URL that act as your health passport for accessing your health information by health provider/hospital online.

Printing your Health Card

Health Card is like your other wallet card which will have information to access you health information .This will have front and back side that can be printed as a card and kept in your wallet.

Steps to print :

1. Go to https://www.healthpassportonline.com/
2. Click ‘Login’ button and enter your credentials
3. Click ‘Health Card’ in the top menu
4. Click ‘Download ‘ button and print the pdf

Accessing your health info from health card :

• By entering health URL
• By scanning ‘QR ‘ code in the back page

Downloading your Health Record

Once you filled the details like health conditions, allergies, medications and other health details, you can generate your health record and download it.

Steps to download health record :

1. Go to https://www.healthpassportonline.com/
2. Click ‘Login’ button and enter your credentials
3. In the dashboard click ‘Download’ button on the top menu
4. Your health record will be downloaded to your local system
   

Accessing your Health Record on Emergency

On an emergency situation, health record should be accessible by primary care provider without patient’s intervention for a better care. Health passport provides emergency access using health URL with passcode provided in the health card.

Steps to access heath record on emergency :

1. Access the Health URL by using QR code present in the patient’s health card or entering health URL directly in the browser
   
2. Select the option ‘passcode’ in the dialog
3. Enter the “passcode” present in backside of patient’s Health card
   
4. Health record will be shown which can be downloaded/printed

Pushing Health Card to Apple Wallet

Apple Wallet is a mobile app included with the iOS operating system that allows users to store Wallet-passes. Health passport allows you to push your health card in apple wallet.

Steps to push health record:

1. Go to https://www.healthpassportonline.com/
2. Click ‘Login’ button and enter your credentials
3. In the dashboard click ‘Health Card’ button on the top menu
4. In the card page click the ‘Add to Apple Wallet’ button present in the top right corner
   
5. Click the saved .pkpass file .In the pass shown click ‘Add’ button to proceed.
6. Check your wallet and you can find the ‘Health card’ as pass in your wallet
   

Accessing Short URL by Caregiver

On an emergency situation ,health record should be accessible by care giver/relative without my intervention for a better care .Health passport provides emergency access using health URL with passcode provided in the health card or OTP sent to my mobile number.

Steps to access my health record :

1. Enter the short URL in your health card
2. Select ‘OTP’ option in the dialog
3. Enter verification code sent to your mobile
   
4. Health record will be shown which can be downloaded /printed
