Core 5 : Active Medication List

Measure Overview : Record active medications of patients.

Denominator : Number of unique patients seen by the EP during the EHR reporting period.

Numerator : Number of patients in the denominator who have a medication (or an indication that the patient is not currently prescribed any medication) recorded as structured data.

Stage 1 Goal : 80%

Exclusion : None

ChARM EHR Workflow :
Follow the steps below to record active medications of patients.

Go to Patient Details > Medications section.
Use 'Add Medication' link to record patient's active medications.
If there is no medication to record then choose 'No Active Medication'
