Core 3 : Maintain Problem List

Measure Overview : Maintain up-to-date problem list of patients.

Denominator : Number of unique patients seen by the Provider during the EHR reporting period.

Numerator : Number of patients in the denominator who have at least one diagnosis entry (or an indication that no problems are known for the patient) recorded as structured data in their problem list.

Stage 1 Goal : 80%

Exclusion : None

ChARM EHR Workflow :
Follow the steps below to record active diagnoses of patients.

1. Go to Patient Details > Medical History section.
2. Use 'Add Diagnoses' link to record patient's active diagnoses.
3. If there is no diagnosis to record then choose 'No Active Diagnosis'.
