Clinical Quality Measures
Measure Description: Percentage of patients 18-75 years of age with diabetes who had hemoglobin A1c > 9.0% during the measurement period
Denominator: Patients 18-75 years of age with diabetes with a visit during the measurement period
Numerator: Patients whose most recent HbA1c level (performed during the measurement period) is >9.0%
Measure Calculation in ChARM EHR: AUTOMATIC
This measure is automatically calculated based on the HbA1c result of the patient.
Lower numerator score for this measure indicates better quality. Hence to satisfy this measure, for patient with Age >= 18 and < 75 years with an active diagnosis of ‘Diabetes’ (Example ICD 10 codes : E10.10, E10.21, E10.29 ), perform HbA1c test for patients and take necessary clinical measures to keep the level below 9%.
Note: Sample LOINC codes for HbA1C test parameter are 17855-8, 17856-6, 4548-4. Before start recording the result, make sure you have updated the LOINC code '17855-8' for HbA1C test parameter under 'Settings > Labs > Customize Labs' section. While entering HbA1C lab result for patient, choose this particular parameter (that has loinc code configured in settings) and input the value, otherwise it will not be considered for CQM calculation.
Denominator Exclusion: You can manually exclude a patient from denominator, based on the conditions listed below under 'Encounter > MU' section.
