CMS123 - Diabetes: Foot Exam

Measure Description: The percentage of patients 18-75 years of age with diabetes (type 1 and type 2) who received a foot exam (visual inspection and sensory exam with mono filament and a pulse exam) during the measurement year

Denominator: Patients 18-75 years of age with diabetes with a visit during the measurement period

Numerator: Patients who received visual, pulse and sensory foot examinations during the measurement period

Measure Calculation in ChARM EHR: MANUAL

This measure is manually calculated based on the actions recorded under 'Encounter > MU' section.

To satisfy this measure, for patient with Age >= 18 and < 75 years with an active diagnosis of ‘Diabetes’ (Example ICD 10 codes : E10.10, E10.21, E10.29 ), perform Visual, Sensory and Pulse examination of the foot and record them under 'Encounter > MU' section.

Denominator Exclusion: Patients having Diagnosis of 'Bilateral amputation of leg below or above knee' (Sample IDC10 Codes : Q72.03, Q72.23 ) are automatically excluded from denominator.

In addition, you can manually exclude a patient from denominator, based on other conditions listed below under 'Encounter > MU' section.
