CMS125 - Breast Cancer Screening

Measure Description: Percentage of women 50-74 years of age who had a mammogram to screen for breast cancer

Denominator: Women 51-74 years of age with a visit during the measurement period

Numerator: Women with one or more mammograms during the measurement period or the 15 months prior to the measurement period

Measure Calculation in ChARM EHR: MANUAL

This measure is manually calculated based on the actions recorded under 'Encounter > MU' section.

To satisfy this measure, for female patient with Age >= 51 and < 74 years, Mammography diagnostic study should be performed at least once in 27 months before end of the measurement period and the action needs to be recorded under 'Encounter > MU' section.

Denominator Exclusion: Patients will be automatically excluded from denominator, if they have Diagnosis ‘History of bilateral mastectomy’ (Sample ICD10 code : Z90.13).

In addition, you can manually exclude a patient from denominator, based on the conditions listed below under 'Encounter > MU' section.
