CMS134 - Diabetes: Medical Attention for Nephropathy

Measure Description: The percentage of patients 18-75 years of age with diabetes who had a nephropathy screening test or evidence of nephropathy during the measurement period

Denominator: Patients 18-75 years of age with diabetes with a visit during the measurement period

Numerator: Patients with a screening for nephropathy or evidence of nephropathy during the measurement period

Measure Calculation in ChARM EHR: AUTOMATIC and MANUAL

This measure is calculated based on the patient diagnoses and actions performed under 'Encounter > MU' section.

To satisfy this measure, for patient with Age >= 18 and < 75 years with active diagnoses of ‘Diabetes’ (Example ICD 10 codes : E10.10, E10.21, E10.29 ) should match any one of the interventions listed below.

Note: Sample LOINC codes for Urine Protein test are 11218-5, 13992-3. Before start recording the result, make sure you have updated correct LOINC codes for Urine Protein test parameter under 'Settings > Labs > Customize Labs' section. Otherwise the result will not be considered for CQM calculation.

Denominator Exclusion: You can manually exclude a patient from denominator, based on other conditions listed below under 'Encounter > MU' section.
