Clinical Quality Measures
Measure Description: Percentage of patients aged 18 years and older with a diagnosis of primary open-angle glaucoma (POAG) who have an optic nerve head evaluation during one or more office visits within 12 months
Denominator: All patients aged 18 years and older with a diagnosis of primary open-angle glaucoma
Numerator: Patients who have an optic nerve head evaluation during one or more office visits within 12 months
Measure Calculation in ChARM EHR: MANUAL
This measure is calculated based on the actions recorded under 'Encounter > MU' section.
To satisfy this measure, for patient with Age >= 18 years with an active diagnosis of “Primary Open-Angle Glaucoma" (Example ICD10 code : H40.10X0, H40.10X1, H40.10X2 ), Cup to Disc Ratio and Optic Disc Exam studies need to performed and recorded under 'Encounter > MU' section.
Denominator Exception: You can manually exempt a patient based on the conditions listed below in 'Encounter > MU' section.
