Clinical Quality Measures
Measure Description: Percentage of patients 3-17 years of age who had an outpatient visit with a Primary Care Physician (PCP) or Obstetrician/Gynecologist (OB/GYN) and who had evidence of the following during the measurement period. Three rates are reported.
Denominator: Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP) or an obstetrician/gynecologist (OB/GYN) during the measurement period
Numerator 1: Patients who had a height, weight and body mass index (BMI) percentile recorded during the measurement period
Numerator 2: Patients who had counseling for nutrition during a visit that occurs during the measurement period
Numerator 3: Patients who had counseling for physical activity during a visit that occurs during the measurement period
Measure Calculation in ChARM EHR: AUTOMATIC and MANUAL
Numerator 1 of this measure is automatically calculated in ChARM EHR based on Height and Weight recorded in vitals section. Numerator 2 and 3 are calculated based on the manual entry under 'Encounter > MU' section.
To satisfy this measure, for patient with Age >= 3 and < 17 years always record Weight/Height in the vital section. Also, provide counseling for Nutrition and Physical activities and record them under 'Encounter > MU' section.
Denominator Exclusion: Patients having Diagnosis of 'Pregnancy' (Sample IDC10 Codes : O00.1, O09.212, O09.522 ) are automatically excluded from the denominator.
In addition, you can manually exclude a patient from denominator, based on other conditions listed below under 'Encounter > MU' section.
