CMS156 - High-Risk Medications in the Elderly

Measure Description: Percentage of patients 65 years of age and older who were ordered high-risk medications. Two rates are reported.

Denominator: Patients 65 years and older who had a visit during the measurement period

Numerator 1: Patients with an order for at least one high-risk medication during the measurement period

Numerator 2: Patients with at least two orders for the same high-risk medication during the measurement period

Measure Calculation in ChARM EHR: AUTOMATIC

This measure is automatically calculated in ChARM EHR based on the High Risk medications prescribed to elderly patients.

Lower numerator score for this measure indicates better quality. Hence to satisfy this measure, restrict prescribing high risk medications (Example: Estrogens, Acetaminophen, Hyoscyamine etc.) to patients with Age >= 65 years.

Denominator Exclusion: You can manually exclude a patient from denominator, based on conditions listed below under 'Encounter > MU' section.
