CMS2 - Preventive Care and Screening: Screening for Clinical Depression and Follow-Up Plan

Measure Description: Percentage of patients aged 12 years and older screened for depression on the date of the encounter using an age appropriate standardized depression screening tool AND if positive, a follow-up plan is documented on the date of the positive screen

Denominator: All patients aged 12 years and older before the beginning of the measurement period with at least one eligible encounter during the measurement period

Numerator: Patients screened for depression on the date of the encounter using an age appropriate standardized tool AND if positive, a follow-up plan is documented on the date of the positive screen

Measure Calculation in ChARM EHR: MANUAL

This measure is calculated based on the actions recorded under 'Encounter > MU' section.

To satisfy this measure, for patient with Age >= 12 years, conduct depression screening test and record the result under 'Encounter > MU' section. If the result is positive, perform any one of the additional interventions listed below.

Denominator Exclusion: Patients will be automatically excluded if the Patient has an active Diagnosis of 'Depression diagnosis’ (Sample ICD10 codes : F01.51, F32.89, O99.342) or ‘Bipolar Diagnosis’ (Sample ICD10 codes : F31.10, F31.70, F31.9)

Denominator Exception: You can also manually exempt a patient from denominator, based on the other conditions listed below under 'Encounter > MU' section.
