Clinical Quality Measures
Measure Description: Percentage of patients aged 18 years and older seen during the reporting period who were screened for high blood pressure AND a recommended follow-up plan is documented based on the current blood pressure (BP) reading as indicated
Denominator: All patients aged 18 years and older before the start of the measurement period with at least one eligible encounter during the measurement period
Numerator: Patients who were screened for high blood pressure AND have a recommended follow-up plan documented, as indicated if the blood pressure is pre-hypertensive or hypertensive
Measure Calculation in ChARM EHR: AUTOMATIC and MANUAL
This measure is automatically calculated based on the BP values in the Vitals section and the interventions recorded under 'Encounter > MU' section.
To satisfy this measure, for patient with Age >= 18 years and diagnosed as pre-hypertensive (Systolic <140 and Diastolic <90) perform below interventions.
For patient with Age >= 18 years and diagnosed as first hypertensive (first time the Systolic level is above140 and Diastolic level is above 90) perform below interventions.
For patient with Age >= 18 years and diagnosed as second hypertensive (second time the Systolic level is above140 and Diastolic level is above90) perform below interventions.
Denominator Exclusion: Patients will be automatically excluded if the Patient has an active diagnosis of 'Hypertension' (Sample ICD10 codes : H35.039, I13.11, I67.4)
Denominator Exception: You can manually exempt a patient from denominator, based on other conditions listed below under 'Encounter > MU' section.
