CharmHealth EHR-PointClickCare Integration

PointClickCare (PCC) is a cloud-based platform designed for Long-Term Post-Acute Care (LTPAC) Facilities. It is widely adopted across skilled nursing facilities, senior living communities, and home health agencies.

CharmHealth is integrated with PointClickCare to streamline Patient Care by automatically synchronizing Patient information and clinical notes between the CharmHealth EHR and LTPAC Facilities.

Onboarding Process

The following steps outline how to enable the integration between CharmHealth EHR and PointClickCare.

1. Actions to be Performed by the PointClickCare Clients

The PointClickCare Clients/Facilities need to raise the integration request to PointClickCare to enable CharmHealth integration by following the steps below.

  1. Log in to the PointClickCare portal.
  2. Go to 'Resource Hub > Customer Support Portal'
  3. Search for 'Self-serve Activation for Marketplace Applications for Quick Start'
  4. Complete and submit the Activation Request Form to authorize the data sharing.

2. Actions performed by CharmHealth

  • Once the activation request is submitted, CharmHealth receives an email notification from PointClickCare.
  • CharmHealth verifies the Facility details and acknowledges the request to PointClickCare for approval.

3. Actions performed by PointClickCare

  • PointClickCare reviews and approves the integration request
  • The approval process typically takes 2-3 business days.

4. Enabling Integration in CharmHealth

  • After approval, CharmHealth configures the integration for the corresponding Facility.
  • Once configured, you can view the active integration under the 'Settings' section of your CharmHealth EHR account.

  • Practices can add Prescribers whose data should be synchronized with the connected LTPAC Facility.
  • You can choose where to place the clinical notes in PointClickCare by clicking the 'Edit' icon and selecting the desired document category if configured for the Facility.

Your LTPAC Facility will then be ready for seamless data sharing between CharmHealth and PointClickCare.

Patient Information Synchronization

When a Patient is created or updated in the PointClickCare system at the LTPAC Facility, the details are automatically synchronized with CharmHealth EHR and can be viewed under the Patient dashboard of the respective Facility.

Transferring Chart Notes of Consulting Provider

  1. The Consulting Prescriber checks the up-to-date Patient details in CharmHealth EHR and documents the clinical Encounter.

  2. After the Chart Note is signed, the visit summary is shared with PointClickCare automatically in two modes:

    a. Progress Notes: The visit summary is added to the Progress Notes section of PointClickCare.

    b. PDF Document: The consulting summary is shared as a PDF and placed under the configured Document Category section in PointClickCare.
