Public Health and Clinical Data Registry Reporting

a) Immunization Registry Reporting

The EP is in active engagement with a PHA to submit immunization data and receive immunization forecasts and histories from the public health immunization registry/immunization information system (IIS).

ChARM EHR Workflow:

ChARM EHR connects to all state immunization registries in USA. Send an email to support@charmhealth.com to enable this feature for providers. It is a bi-directional integration. Providers can submit the patient’s vaccine data to registry, as well query patient’s immunization history/forecast from registry records.

b) Syndromic Surveillance Reporting

The EP is in active engagement with a PHA to submit syndromic surveillance data from an urgent care setting.

ChARM EHR Workflow:

At this time, ChARM EHR does not support electronically submission of syndromic surveillance data. You can generate HL7 Surveillance Report from Encounter section of ChARM and do a manual submission.

c) Electronic Case Reporting

The EP is in active engagement with a PHA to submit case reporting of reportable conditions.

ChARM EHR Workflow:

At this time, ChARM EHR does not support electronic case reporting.

d) Public Health Registry Reporting

The EP is in active engagement with a PHA to submit data to public health registries.

ChARM EHR Workflow:

At this time, ChARM EHR does not support public health registry reporting.

e) CDR Reporting

The EP is in active engagement to submit data to a CDR.

ChARM EHR Workflow:

At this time, ChARM EHR does not support CDR reporting.
