MU Stage 1 - Core Measures

Core 8 : Record Vital Signs

Measure Overview : Record blood pressure for patients ages three and above. Record height and weight for patients of all ages.

Denominator : Number of unique patients seen during the reporting period.

Numerator : Number of patients in the denominator who have at least one entry of their height, weight and blood pressure recorded as structured data in the EHR.

Record Vital Signs (Height/Weight not in scope)

Denominator : Number of unique patients ages 3 and over seen during the reporting period.

Numerator : Number of patients in the denominator with at least one entry of blood pressure recorded as structured data.

Record Vital Signs (BP not in scope)

Denominator : Number of unique patients seen during the reporting period.

Numerator : Number of patients in the denominator who have at least one entry of height and weight recorded as structured data.

Exclusion : Any provider who

  • Sees no patients 3 years or older is excluded from recording blood pressure
  • Believes that all three vital signs of height, weight, and blood pressure have no relevance to their scope of practice is excluded from recording them
  • Believes that height and weight are relevant to their scope of practice, but blood pressure is not, is excluded from recording blood pressure
  • Believes that blood pressure is relevant to their scope of practice, but height and weight are not, is excluded from recording height and weight.

Stage 1 Goal : 50%

ChARM EHR Workflow : Follow the steps below to record patient's vital signs.

  1. Open new encounter from 'Patient Dashboard' or 'Calendar > List View'.
  2. Choose the required Chart Type.
  3. Go to Health Vitals section.
  4. Enter patient's weight, height and BP.
  5. Save the details.
