Creating Templates

Create Heading

Drag and Drop the 'Heading' component to the section on your right. You can enter the name of the heading and customize its attributes viz., font size, bold, italic, underline, etc. Click on the 'OK' button to add the heading component.

Create Checklist

  • Drag and Drop the 'Check List' component to the section on your right.
  • Enter the options that you need in the checklist.
  • You can create hierarchical values by using the '>>' symbol followed by the option value.
  • You can also customize how the generated text would look upon filling the component. You can use the placeholders like '{question}' or '{answer}' to customize the generated text. Click on the 'OK' button to add the checklist component.

Simple Question

  • Drag and Drop the 'Simple Question' component to the section on your right.
  • Enter the question in the 'Question' field.
  • If the answer runs for a larger value, click on the 'Multiline answer' checkbox so that the text area will appear big.
  • You can also customize how the generated text would look upon filling the component.
  • You can use placeholders like '{question}' or '{answer}' to customize the generated text. Click on the 'OK' button to add the component.

Single Choice Component

  • Drag and Drop the 'Single Choice' component to the section on your right.
  • Enter the question in the 'Question' field.
  • In the 'Options' field, enter the values for the choices, one in a line. You can create hierarchical values by using the '>>' symbol followed by the option value.

Multi Choice Component

  • Drag and Drop the 'Multi Choice' component to the section on your right.
  • Enter the question in the 'Question' field.
  • In the 'Options' field, enter the values for the choices, one in a line.

  • You can also customize how the generated text would look upon filling the component.
  • You can use placeholders like '{question}' or '{answer}' to customize the generated text. Click on the 'OK' button to add the component.

Rating Scale

  • Drag and Drop the 'Rating Scale' component to the section on your right.
  • Enter the question in the 'Question' field.
  • Define the range for the rating scale in the 'Min' and 'Max' fields. You can use placeholders like '{question}' or '{answer}' to customize the generated text.
  • Click on the 'OK' button to add the component. For example, for pain level, you can enter the minimum to be 1 and the maximum to be 10.

Yes/No Question

  • Drag and Drop the 'Yes/No Question' component to the section on your right.
  • Enter the question in the 'Question' field. You will get the options 'Yes' and 'No' by default.
  • You can also customize how the generated text would look upon filling the component.
  • You can use the placeholders like '{question}' or '{answer}' to customize the generated text.
  • Click on the 'OK' button to add the component.

Notes Component

  • Drag and Drop the 'Notes' component to the section on your right.
  • This option allows you to enter text in a text area during the Consultation.
  • Click on the 'OK' button to add the component.

Table Component

  • Drag and Drop the 'Table' component to the section on your right.
  • Choose the number of columns to be created.
  • Enter the heading in the cells where you would like to have the default header.
  • You can add as many rows as you need using the '+ Add Row' option.
  • Click on the 'OK' button to add the component.

Image Component

  • Drag and Drop the 'Image' component to the section on your right.
  • You can annotate on the uploaded image during the Consultation.
  • Browse the image to be uploaded and click on the 'OK' button to create the image component.
  • Generic anatomical images of the body can be uploaded using this option.
