CharmHealth EHR allows physicians to share pre-appointment questionnaires and consent forms with the patients, when the patient requests for an appointment. Patients can fill up these forms from their Charm PHR account. Secure message notifications are sent to physicians when the patient fills up the forms. This enables the provider to know the purpose of the patient's visit and the scope of their concerns, thus allowing them to review the patient's data before the appointment.
Pre-appointment questionnaires allows patients to enter the details about their demographics, history of present illness, active list of medications being taken, etc. These information helps providers to take informed decisions about the patient health.
