If an Appointment request is sent from the PHR or Practice Website, then it needs to be confirmed. If the request is not pre-confirmed, refer to the 'Settings > Calendar > Online Appointments' section to check whether the Appointment is set to be pre-confirmed or not.
The Pre-screening form filled by the Patients is accessible from the 'Calendar > Appointment Requests' section.
Click on the 'Appointment Requests' option to view the details filled in by the Patient before confirming the Appointment request.
Once the Appointment is confirmed, you can access the filled Pre-screening form from the 'Patient Dashboard > Patient Details > Questionnaires' section.
