Manage Account

My Family
Click on the 'Manage Account' button to view the following tabs:

  1. The 'My Family' tab displays linked family members and their details.
  2. The 'My Contact' tab holds the Practice, health, and wellness-related contacts you add.
  3. The 'Audit Logs' tab tracks your entire account activity.
  4. The 'Account Info' tab displays general account-related information about you.
  5. You can exercise the 'Change Password' feature as need be.

Note: You can edit or delete data in all the tabs under 'Manage Account' except the 'Audit Logs' tab.

Change Your Email Id

Users can update their PHR account email ID using the following steps:

  1. Select your profile picture or username on the PHR dashboard.
  2. Click on the 'Manage Account' button.
  3. Choose the 'Accounts Info' tab.
  4. Select the 'Edit' button against your mail address.

  5. Enter your account password, if required.
  6. Click on the '+Add Email Address' link under the 'My Email Addresses' section.

  7. Enter the new email address and click the 'Add' button.
  8. Fill in the OTP received in the new mail ID. Click on the 'Verify' button to complete the addition.

Users can view all email addresses under the 'My Email Addresses' section. Hover over each mail ID to set it as your primary mail address or delete it.

Note: You may have to verify yourself from time to time while adding a new mail address. Enter your PHR account password when requested.

Setup Two-Factor Authentication

  1. Sign in to your PHR account
  2. Visit this link https://accounts.charmtracker.com
  3. Select the 'Multi-Factor Authentication' tab.
  4. Set up your two-factor authentication either with OTP SMS or authenticator applications.

Note: You may have to verify yourself to make these changes to your account. Enter your PHR account password when asked.

Change Patient Profile

  1. Select your profile picture or username in the PHR dashboard.
  2. Click on the 'Manage Accounts' button
  3. Select the 'My Family' tab
  4. Click on the 'Edit' button against the patient's name.

  5. Make the required updates for the said person.
  6. Click on the 'Update' button to save the changes.

Note: These changes will not reflect in the patient's EHR records with the Practice.

Change Password

If you remember your old password,

  1. Sign in to your PHR account
  2. Select your profile picture or username in the PHR dashboard.
  3. Click on the 'Manage Accounts' button
  4. Select the 'Change Password' tab

  5. Enter your old and new passwords.
  6. Click on the 'Update' button.

If you do not remember your old password,

  1. Visit this link https://accounts.charmtracker.com
  2. Click on the 'Forgot Password' option
  3. Enter your PHR account's mail address (CAPTCHA, if any).
  4. Click on the 'Next' button

  5. Select the 'Continue to Reset Password' option
  6. You will receive the OTP via mail. (Enter your mail ID once again, if required)
  7. Enter the two-factor authentication OTP if enabled.
  8. Enter the new password and click on the 'Change Password' button.

Note:

  1. Check the SPAM folder if have not received password reset email.
  2. Password is case sensitive. Hence make sure that you enter the password in proper case.
  3. For your reference, account username is mentioned in the password reset email.

Adding a Family Member

Users can add multiple family members to their PHR account using the following steps:

  1. Have your family members update the same email ID as you with their Practices. (These Practices should also be Charm registered)
  2. Click on the Family Member's Practice invite link.
  3. Enter their date of birth (mm-dd-yyyy format).

  4. Click on the 'Next' button to complete their user verification.
  5. Enter the same 'Username or Email ID' and 'Password' you created previously.
  6. A warning message with the 'Patient Name' confirming their addition to your PHR account will pop up.

  7. Click on the 'Submit' button.

Note: The linked family members can view and access each other's clinical information within the account.

Adding a Practice

  1. Submit the same email-id to the new Practice or Facility registered with Charm.
  2. Click on the invite link sent to your mail.
  3. Fill in your date of birth (mm-dd-yyyy format).

  4. Click on the 'Next' button to complete the user verification.
  5. Enter the same 'Username or Email ID' and 'Password' you created previously.
  6. A warning message with the 'Practice Name' confirming the addition will pop up.

  7. Click on the 'Submit' button.

Errors in adding a family member or practice

Once you click on the 'Submit' button, Charm will verify your details. The following tips will help you avoid verification errors:

  • Users can complete the verification process only if they have logged into their existing PHR accounts.
  • An error message appears when the log-in mail ID is not associated with the Practice invite URL.
